Please direct your staff to use the “First Available Appointment” option on this form to schedule your patients in an expedited fashion. Our goal is to confirm your patient’s appointment or needed procedure in a timely fashion to assure continuity of care is delivered from your practice to ours.
When your patients schedule an appointment with our physicians, we ask that they bring all current medications and their most recent films to their appointment. Although delivery of x-rays to our office from imaging facilities is sometimes available, we strongly recommend that the patient obtain copies of the the necessary studies, and can be responsible for their safe transport. If there is a special circumstance, please alert our staff and we will be happy to assist.
